In order to make an initial appointment, please contact the Intake Department at 708 586 7357 or at email@example.com. Please leave a message indicating your name, telephone number, and which therapist you would like to work with. We will return your call and assist you in scheduling a first appointment within 1 business day.
We are happy to call your insurance company in order to obtain a quote of your benefits, so that you will know ahead of time approximately what your out of pocket cost will be. However, the information that insurance companies give to patients directly may, at times, be more up-to-date and accurate than the information the insurance company gives to a provider; therefore, we strongly encourage you to call your insurance representative for a quote of benefits, as well.
Copays and coinsurance payments are always due at the time of service and can be remitted via check or credit card. The initial diagnostic session is billed at $225. Thereafter, sessions are billed at $150 for 45-minute sessions, and at $200 for 60-minute sessions.
After your initial appointment, we will submit your claim to your insurance company (we are in network with BCBS PPO and Blue Choice ONLY but are happy to help you submit out of network claims). Once your insurance company has processed your claim (this may take several weeks to a month), our office manager Jade will mail you a statement for any balance that may remain due to deductibles not being met. Statements usually go out during the first week of the month. Please be aware that we typically do not send out statements if your account does not carry a balance; if you would like a statement for your records, please let your clinician know and we will be happy to provide one.
Please be sure to you arrive a few minutes early for your initial appointment to allow sufficient time to complete registration paperwork, which you’ll find on clipboards in the waiting area.

Due to confidentiality concerns, we do not have a receptionist at the office. When you arrive, please have a seat in the waiting area and your clinician will meet you at your appointment time.
